Form W-9

All referees must submit a W9 each year. The process for submitting W9s will be done digitally. You will receive an email after registration to fill out your W9. You will fill out, sign and send the document back via Dropbox Sign. If you move during the season, you must request an updated W9 to fill out.

For the 2023-2024 season, schools now have 30 days to pay officials. After 30 days, please submit the pay issue form below.

For School Administrators:
